Transcript Requests

If you are a graduate/alumni of Monomoy Regional High School, Harwich High School, or Chatham High School, please complete this form to request a transcript. 

In order for the transcript to be considered official, the transcript needs to be sent directly from one institution to another. You may also request that an unofficial transcript be sent directly to you. 

Please note that transcripts are sent via email. If a hard copy is required by mail, please specify that in the comment field. 

  • Please allow 3-5 business days for processing during the school year. 
  • Processing time may be longer during school vacations and over summer break. 
  • An email confirmation will be sent to the requester once the transcript has been sent.
